PPlease reach out to me directly whenever possible, although I understand that, in certain circumstances, making a non-anonymous report can be complicated. Please complete this form if you perceive or experience:
- Discrimination
- Harassment
- Bullying
- Retaliation
- Failure to ensure equal treatment
- A conflict of interest
- Failure to accommodate special needs or issues with accessibility
Please provide as much information as you consider necessary and are able to share. After receiving the report, the Commissioner will review it. However, as the report is submitted anonymously, its handling cannot be guaranteed.
